Nomad, Not Naive: My Honest Take on Healthcare & Insurance on the Road
It took me exactly 37 minutes into my first solo trip to realize I had no idea what I’d do if something actually happened to me.
I was standing at the Chiang Mai airport with a backpack, a remote job, and a health insurance plan that, it turned out, didn’t even cover Thailand.
Lesson learned.
Since then, I’ve tested a half dozen insurance plans, read the fine print most people skip, and had to make one middle-of-the-night hospital visit (everything’s fine—just dehydration and a massive lesson in preparation).
So here’s my non-corporate, non-overwhelming list of healthcare + insurance options I genuinely recommend to other nomads.
Whether you’re in Bali, Berlin, or bouncing between both—these are the ones that actually make sense for our lifestyle.
1. SafetyWing – The One Most People Start With
Low monthly price ($45ish)
Covers most countries, emergency care, and even COVID
You can start it after you’ve left your home country
No-frills, but does the job
My take: If you're early in your nomad journey, this is the least stressful option to set up. Think of it like your "starter pack" for global living.
2. Insured Nomads – The Tech-Savvy Upgrade
More bells and whistles (mental health, apps, safety tools)
Clean user interface, built for long-term travelers
Includes coverage for remote work support, lost gear, and evacuation
My take: I like this one for digital entrepreneurs or if you're part of a remote team. It's comprehensive but not overwhelming.
3. World Nomads – For the Adventurous Soul
Covers over 150 activities (surfing, hiking, scuba—you name it)
Trip-based pricing (you choose the location + duration)
Great reputation in the backpacker and adventure traveler communities
My take: Ideal if you're traveling hard and fast. I wouldn’t use it for slow madding, but for 3-month treks, it’s gold.
4. IMG Global – The Customizer’s Dream
Options for both emergency and comprehensive coverage
Flexible deductible choices
Can feel a little old-school to set up—but it's solid
My take: This is for the spreadsheet lovers who want to tweak and tailor everything. Worth it if you want exact control.
5. Cigna Global – The Corporate-Expat-Level Option
Full medical insurance, like what you'd get from a company job
Excellent international hospital access
More expensive, but truly comprehensive
My take: Best if you're staying in one country long-term or you're running a remote business and want consistent, high-quality care.
6. Allianz Global Assistance – The Reliable Classic
Global brand, known for trip protection
Can add travel disruption and cancellation cover
Feels a bit… traditional, but trustworthy
My take: I pair this with other coverage when I’m doing a high-cost trip (think: a safari or Patagonia). It's great for cancellations and delays.
7. Aetna International – Premium Peace of Mind
Telehealth, prescriptions, maternity, mental health, chronic care
U.S.-style full insurance, but for global expats
Very premium pricing
My take: For those who can afford it and want true “set-it-and-forget-it” global health coverage. Overkill for casual nomads, but great if you're abroad indefinitely.
8. GeoBlue – Built for Americans Abroad
Medical plans with access to U.S. hospitals and care
Great if you're bouncing between the U.S. and abroad
Some plans require U.S. residence
My take: If you’re a U.S. passport holder but spend time home every year, this is a way to stay covered without gaming the system.
9. Integra Global – Designed for Mental Wellness Too
Strong focus on mental health and well-being
Covers outpatient treatment and therapies
Especially good for solo remote workers
My take: Love this one for its holistic mindset. If your work relies on you being emotionally and mentally sharp, it’s a great match.
10. NOMAD Health (by Passport Card) – Real-Time Payouts
You get a prepaid debit card to pay for treatment instantly
No wait to be reimbursed
Smart if you want to avoid paperwork or upfront cash
My take: This is ideal if you've ever sat in a foreign hospital worrying how you’re going to pay. Super efficient system.
My thoughts on healthcare
I used to think “I’ll figure it out if something happens” was part of the freedom of this lifestyle.
It’s not.
What’s free? Being able to say yes to last-minute trips, surf lessons, and deep work sprints without wondering what happens if you twist your ankle—or your appendix gets ideas.
A good insurance setup is like a power bank for your lifestyle: you might not need it every day, but when you do, you're glad it’s charged.
